Sports Physicals

Hello! I hope this email finds you well! Due to COVID, many things have changed. At Central, we are not immune to this. Unfortunately, we will not be able to have our Free Sports Physical Clinic. The Back to School Fair will also not be having their health portion of the fair, which included physicals, dental exams and immunizations.

Here are a few other options for you to get your athlete ready to play school sports:

East Adams Clinic

- July 9^th 1:00-5:00 p.m. Call (217) 696-4446 Appointments required, walk-ins will not be seen.

Quincy Medical Group (appointment required, face covering required, one adult per athlete)

- Visit quincymedgroup.com/sports to download the pre-participation form
